Personal Narrative: Reporting Party
Reporting party (RP) stated that she came to visit her grandmother, Violet Cavallo (age 93, DOB: 12/7/23) who is on hospice on 2/18/17 and 2/19/17. RP lives in Oregon. When she arrived at 10:30 am on 2/18/17, she found resident slumped over in her wheelchair; her chin resting on her chest, sleeping. RP woke her up, she notice that the resident could not change her position; she was slumped over to her left side and her neck was tilted at a slightly unnatural angle. RP asked staff if there was something that could be done about her posture and one of the caregivers brought a pillow and stuffed it between the armrest of the wheelchair and her body. RP arrived at 12:30pm the next day to find the resident slumped over as she found her the previous

    The next, January 28, 2013 (Monday), we are scheduled to visit the MAYNILAD WATER TREATMENT PLANT. We arrived at 8:30 o’clock in the morning at MAYNILAD. But before we entered to the plant, all of us were registered to the guard’s log book and we make sure that we are wearing our hard hat for safety purposes. Then Engr. Bautista, (one of the staff of the plant) discussed us about the company profile, the processed and the equipment they used in treating the water. With this, we know that the MAYNILAD is owned by Metropolitan Waterworks and Sewerage System (MWSS). They are the services provider for the 17 cities and municipalities that comprise the West Zone of the greater Metro Manila area (540 square kilometers) and almost a million of consumers. They have two major source: The Angat Dam and the Laguna Lake. They get almost 97% of water from Angat dam. The plant consists of seven processes, namely, screening, coagulation/flocculation, sedimentation/clarification, filtration and disinfection.…

    A new sunny morning come for my life. I woke up early as 5:30 in the morning. Monday, I prepared myself and even my bag bringing it every go to office. At 7:40 in the morning, I left from home.
