The Influence Of Patient Dressing
I remember us having several staff meetings to address situations where a certified nurse aide had changed a patient dressing without asking or notifying the LPN or RN. The CNA was giving a patient shower and forgot to cover the patient’s dressing. Therefore, the dressing was wet and she took the initiative to replace the dressing without informing the RN or LPN. There is always a risk involved when practicing outside one’s scope of practice. Based on the example above, if there is a side effect or reaction that happen from the dressing changed by the CNA there will be no backup or documentation to prove that it was properly done. When scene like this happen, it has to be reported and corrected immediately to avoid further complications and/or

    The nurse delegated tasks to aides which was expected. Two patients asked the nurse if she could help them change their brief and the nurse was busy in a med pass and told the patient that she would get their aid. We spent the first hour of the shift going over the diabetic patients and finding out what their sugar levels were to be able to properly prioritize the most critical patient before dinner. The LPN that I shadowed did not fill the expectation of cleanliness however.     Nursing sensitive indicators reflect the structure, process and outcomes of nursing care. The structure of nursing care is indicated by the supply of nursing staff, the skill level of the nursing staff, and the education/certification of nursing staff. Process indicators measure aspects of nursing care such as assessment, intervention, and RN job satisfaction. Patient outcomes that are determined to be nursing sensitive are those that improve if there is a greater quantity or quality of nursing care. ("Nursing world," 2013) Having knowledge of these indicators hard-wired and put into practice in daily care of patients, could have assisted the nurses involved in this particular case in many ways. Knowing the risk of pressure ulcer development with the use of restraints could have helped the nurse prevent one in this patient. The bedside care providers, the nurse, as well as, the CNA would have recognized the reddened area as the first stage in pressure ulcer development. They would have been more meticulous with skin assessments and repositioning with the use of restraints in this elderly patient. Also, if the CNA had a better understanding of when to appropriately use restraints and how to manage a restrained patient she wouldn’t have positioned the patient on his back after he returned to bed. Another nursing sensitive indicator is patient and family satisfaction which was not taken into account in this situation. The nurse so easily dismissed the daughter’s concerns about the dietary mix up and blew off her concerns regarding what her father was served. Nursing sensitive quality indicators are an important part of the equation when it comes to establishing evidence-based practice guidelines. But measuring these indicators is not simply good science – it’s an ethical imperative.…

    The Texas Board of Nursing (BON) implemented Safe Harbor for nurses to have a formal procedure to object to an unsafe situation in the workplace. The nurse’s duty is always to the patient! The BON (2013) refers to the duty of a nurse “always advocate for patient safety, including any nursing action necessary to comply with the standards of nursing practice and to avoid engaging in unprofessional conduct. This includes administrative decisions directly affecting a nurse's ability to comply with that duty.” If a nurse is put in a position where they feel that safe harbor should be implemented the steps afforded to them must be followed explicitly. The BON offers an abbreviated version of the form for safe harbor that can be filled out quickly, but the comprehensive version of the safe harbor form must be filled out before the end of the work period. According the BON (2013) “a nurse invoking Safe Harbor may engage in the requested assignment or conduct while awaiting peer review determination unless the conduct or assignment is one that would constitute unprofessional or criminal conduct, or the nurse lacks the basic knowledge, skills and abilities necessary to competently perform the assignment.” Another step in the Safe Harbor process according to the BON (2013) “the supervisor should sign the form and the nurse’s copy and submit the form to the Peer Review Chairperson since a peer review meeting to review the request must be set within 14 days of the nurse invoking Safe Harbor.” Marshall and Zolierek (2012) state “the Texas Nurse Practice Act requires organizations that employ 10 or more nurses to establish a nursing peer review committee for the purpose of reviewing practice incidents that potentially meet conditions for reporting to the Board of Nursing and to review nurses' requests for “safe harbor” in situations where the nurse believes she/he was in a practice situation with potential risk for violating a duty to the patient” (p. 36).     The phenomenon of cross-dressing is something that people do not completely understand. Almost every city has a subculture of people who dress in the opposite gender’s clothing, either for fetishistic purposes or for means of artistic expression. Although everyone has seen a cross-dresser in some way, people quickly lump cross-dressing, homosexuality, and being transgender together into one giant group, without taking the time to individually understand each aspect and circumstance, as well as lacking to understand what someone is going through; This is something that is dangerous in our society. In my journey in understanding this phenomenon specifically in Los Angeles, I partook in participant observation, observing the people closest to this subculture while becoming friends with some of the most recognized drag performers of the moment, and meeting people who get a sexual thrill when emitting the opposite gender. I observed these individuals in the local nightclubs of L.A. (Micky’s, Rage, Hamburger Mary’s), where crowds gathered to watch their favorite drag performers lip-synch to popular songs, and also entered the homes of others that kept their fetish interests secretive from their closest relatives. In my research, I was interested in understanding whether cross-dressing was a way of escaping societal norms, specifically for men, in order to express themselves more femininely as society oppresses them from doing, or whether it was simply a way of being creative and artistic; I also wanted to understand what the media’s portrayal of cross-dressing is, to better understand their struggles in society due to social construction.
