There are many diverse clinicians that are employed today within the home health aspects of the medical field. Some of these clinicians consist of occupational therapist, physical therapist, respiratory therapist, registered nurse and a licensed practical nurse. Even though the ending result for the home health organization is to rehab the client back to their independence as much as possible, the agencies continue to be a competitor for each other. Each agencies is striving for their clinician to have more knowledge and wisdom the different aspects of live for the client. An example of this could be a clinician knowing how to properly do wound care for the need of the client, or even using the updated technology need to monitor for a decline in the clients physical and mental capacity. …show more content…
Attached is a copy of my skilled nurse performance evaluation that is administered by my present employer annual. The evaluation is designed to target in the clinician weakness and also their strengths. The evaluation is divided in three parts of how the clinician is rated. . These consist of DC (Deemed Competent), AT (Additional Training Needed), and N/A (Does not apply). Deemed competent is the goal of all the clinician in all areas as much as possible.     I think that every aspects of nursing assessment are challenging and it depends on the nurses’ experience. When we have a new admit patient/resident, nursing admission assessment should be completed within 24 hours, and I usually would finish it. Working on the night shift, I find it challenging to complete the assessment because the patient is already tired and wanting to have sleep. I cannot force them to participate, and it is the resident’s right to refuse. When assessing for the Brief Interview of Mini-mental Status (BIMS), it is often not 100% correct due to contributing factors involved, that is being sleepy and tired. When BIMS is done during the day, the result is way different than what I had obtained. To have a new patient in the rehabilitation floor who have frustrations in life might be more challenging because the patient might not participate in the assessment, in his care or just give up.     The nursing process has five key steps in it. There is an acronym to remember these by steps by; it is ADPIE. Assess Diagnose, Plan, Implement, and Evaluate. The assessment step is exactly as it states; a nursing assessment. The nurse assesses the patient and gathers information to make a diagnosis. The next step is diagnosing; in which means forming a nursing diagnosis based on subjective and objective data; and on the patient history. Once a nursing diagnosis is formed; the nurse must plan for patient care and make a care plan for treatment, setting appropriate and measurable goals to be reached. Next is implementation. In this step of the nursing process the nurse implements, or puts into action the plan of care.
