At that moment, I informed him that I could not check him in unless he has his ID card. He replied, “Even if they already know who I am”, I said “yes, even if they know who you are, I cannot check you in without your ID it is protocol”. I proceeded to inform him that if he had another form of identification he can go to medical records and get a promissory note. At which time he became upset and stated he wanted to speak to someone. I immediately, asked Major Stackhouse for assistance, I explained to him the situation and also informed CPT Porter that he had to obtain a promissory note. The heart is what some determine to be the most important organ in our bodies and one of the biggest contributors. It is one of the major organs that if we did not have, it would not be possible for us to live. The heart is about the size of a fist and is broken down into four chambers, the aorta, superior vena cava, pulmonary artery, and the coronary artery. The four chambers include the right and left atrium and the right and left ventricle. The heart is responsible for supplying oxygen and blood to the entire body. Blood passes through these four chambers and then exits and pumps into the rest of the body. The heart also has three layers of walls…

Based on the number of ASC comments that didn’t understand that the 35% Design drawings were only included for reference, the direction on the drawings may not have been clear enough in Performance Spec. After discussing it further, we feel that it would be better to remove non-relevant 35% Design drawings from the Performance Spec package. A note could be added to the PS that explains that the full drawing package was not included, but will be provided after award (or upon formal request).…

If a patient visits a specialist but did not receive the required authorization prior to the visit, the claim may be denied, resulting in the provider’s need to appeal the claim (Jacob, 2001). Healthcare employees who handle billing and claims must be certain that all of the information they have for each patient is correct and up to date, and that they receive all necessary authorizations prior to performing any procedures. Additionally, insurance clerks have to be certain that they are using the proper procedure codes and not unintentionally over coding. Should a claim be denied, no matter the reason, it must follow the three steps of the appeals process. These three steps are complaint, appeal, and grievance. By filing an appeal, the claim can be paid when it was previously denied, reduced, or down coded. After the appeals process and decision, if a provider or patient is still not satisfied, the appeal can be taken to an outside authority, like a state insurance commission…

I have not personally had an animal diagnosed with heart disease at my clinic, where I had to have a conversation with an owner about a diet change, so I did my best with this post to imagine the different things I would want to be sure to tell an owner about heart disease and how I would want to make sure they understood the importance of changing their animal’s diet. One of the first things I would try to do is find out what type of food and treats they were currently giving to their animal and find out the sodium content of that food and treats. This way I could identify the areas they need to change, and I would also be able to see if there is anything they are currently giving to their animal that they could still give occasionally. I…

1.1 Explain the importance of recognising the centrality of the individual rather than the disability.Words are critical in shaping ideas, perceptions and attitudes. They can be used to reinforce existing prejudices or to liberate people from stereotypes, prejudices and misconceptions. If anyone doubts the value of this or the potential for relatively rapid change, consider the distaste with which we all now view the word “nigger”. None of us would dream of using it. Yet only a generation or two ago it was an internationally acceptable and widely used term for the individual people of an entire ethnic group in the USA; not just in use orally, it also appeared in book titles. In more recent times the Australian word “abo” to describe an aboriginal or indigenous person is considered offensive by most people and is dropping out of use. The moral Language changes, and we are capable of giving it a nudge in the right direction - even if we run the risk of being considered “politically correct” by some people or sometimes require a few extra words to make our communication acceptable. In the context of disability, negative and patronising language produces, predictably, negative and patronising images and attitudes. Words in popular use mirror attitudes in society and by changing the words we can begin to change those attitudes. Those attitudes are often the most difficult barriers…

In order for a patient to succeed in a claim for clinical negligence against his doctor, he must be able to satisfy three requirements: first, he must establish that a duty of care was owed by the doctor or hospital to himself; second, he must prove that the doctor has breached that duty of care by failing to reach the standard of care required by the law; lastly, the patient must prove that his injury was caused by the doctor’s negligent act. Each of these requirements for negligence will be considered as the strict requirements for a successful claim of the patient suffering from an adverse event in a medical context. These steps were referred by Charles Foster as the…
