I am currently on placement at the Fred Paton Day Care Centre which is a Centre for elderly service users. My role in the facility is to provide support / assistance when required and to help engage service users in activities that are provided by the staff within the organisation. At present I am working with a 62 year old male who has recently suffered from a bad stroke and another recent mini stroke which has left him with limited mobility making some tasks harder for him.
For the purpose of confidentiality and the Data Protection Act 1998 I will be referring to the service users as John, which is not his real name.
Since recently suffering from a stroke at the age of 61 John has been left with limited mobility in his left hand side of his body including the use of his arm and some use of his right leg. He is a very shy individual who is finding it a struggle to get used to dealing with his condition as he up until his illness was a fit, reasonably healthy man who worked as a scaffolder and had a brilliant social life. As a result of his stroke he feels like he has lost a lot of his independence and that he needs to rely on others to help him with a variety of things including personal hygiene and getting around outside his home. John still lives at home with his wife of 38 years who is still able to provide a level of care for him although he also has care workers come into his home in evenings to prepare him for bed aswell as his daughter who helps out when she can. Although John has accepted that he does not have full mobility he is finding that it has knocked his confidence and he does not like having to rely on others to help him out so much and is very reluctant to use a wheelchair even when he is tired. Johns not one for being very sociable in the day Centre he tends to keep himself to himself and doesn’t feel confident enough to get into a lot of conversations

    1. As a carer your role is not to do things for the individuals you care for but to do the tasks including the service user. A support workers role is to work in partnership with the individual and support their decisions, offer as much or as little assistance the service user requires for their care and support them to do as much as possible for themselves. If you provide too much care for an individual they can lose their skills and independence and not learn any new life skills. By providing support and encouraging the individual to participate in their daily living needs you are helping them to develop and maintain their skills for everyday life. Skills are easily regained if the service user is shown how to complete a task. For example if a service user is having a shower and would usually have his hair washed for him, why not show him how to wash his own hair. Whilst providing personal care for my residents when it comes to hair washing, I normally provide the shampoo and apply the shampoo to the service users palm. I then mimmick washing my own hair and ask the service user to copy me. This is also useful in showing a service user where needs to be washed. I also talk to the service user about the task they are doing, for example I would mimmick washing my arms and say wash your arms that's it, don't forget underneath your arm. This is enforcing new skills as well as maintaining them, it also empowers the service user as they have achieved this task themselves with minimal support.…

    All care workers involved with a service user must be aware of what care is needed for that person and know and understand in order to co-ordinate care efficiently. This can be done by liaising with all parties involved in providing care for the individual, eg the service users themselves this will encourage and support them to be involved in their own care, helping them maintain their skills etc. social worker, family, home care agency, meals on wheels, district nurses, GP. There is no point in a GP stating dressing needs to be changed daily and district nurses go in weekly, or the district nurses go for visit at home and the person is not there because on the day they visit they are at day care. A person may have meals on wheels every day, but then gets a place within day care for 2 days per week, meals on wheels would then need to be informed that meals will not be needed on those days, also when day care is closed for bank holidays meals on wheels would be put back in place for those days, or if home care lunch calls are normally supplied, those would be put in place, this also happens if a person does not attend day care if unwell.     I first attempted to maintain my routine chores at home. I tried to make dinner for my son and it was very difficult. I could not control the frying pan it kept moving all over the burner; I attempted to try and stir the food with my mouth, which was very scary and dangerous. In the end, I relied on my son for some assistance. I tried to fold some laundry and I did not get far, I was making more of a mess. Putting laundry away was very time-consuming, normally it would take me 10 to 15 minutes to finish, but not having the use of my right arm it took 30 to 40 minutes. Starting the car was a challenge; had to find a way to maneuver around the steering wheel to put the key in the ignition. At work, I attempted to get a resident dressed and took me 20 minutes; which would normally only take 5 minutes. A person with hemiplegia would not be able to perform my job the use of two hands is a necessity for efficiency and to maintain the safety of residents. I tried to see if I could prepare a residents tray for breakfast. I could not open the milk carton or pass the tray in a safe manner. I work as a certified nursing assistant (CNA) on an Alzheimer’s unit and the need to have full use of all your extremities is a necessity to care for the residents.
