GORDON’S 11 FUNCTIONAL HEALTH PATTERNS
Values, Health Perception:
How does the person describe their current health?
What values did you learn as a child that are still important to you?
What steps does the family take to improve their health?
Nutrition:
Can they describe what their family’s daily diet consists of?
What kind of foods do they like to eat, what foods do they avoid and why?
Can they describe their weight pattern over the last year?
Sleep, Rest:
Can the person tell you what their sleep habits are?
What do you do when you can’t sleep?
Can the person describe the steps they take to get ready for bed?
Elimination:
What can they tell you about their elimination patterns?
What do they do if they feel constipated?
What kind of fluids and how much do they drink on a daily basis?
Activity, Exercise:
How do they describe theirl weekly pattern of activity and exercise?
What kind of group activities, Teams sports, do they participate in?
What kind of activities do they engage in as a family?

Cognitive:
What level of education have they completed?
What can they tell you about school?, do they find it easy or do they have difficulty with a subject?
What can they tell you about what they did last week?
Sensory-Perception:
Do they have any sensory deficits and are they corrected?
Can they describe their ability to walk, exercise?
What do they like to do for leisure time?
Self- perception
What can they tell you about their emotions and moods?
Can they describe their body image, what they think about themselves?
How do they feel about themselves?
Role Relationship:
How do they describe the family members in relationship to themselves?
What relationships are most important to them?
Who do they describe as being a good role model for them?
Sexuality:
Are they satisfied with their situation related to their sexuality?
Do they have any problems with their reproductive system?
What can they tell you about relationships with the opposite sex?
Coping:
How do you cope with problems?
Do these actions help or do they make the problem worse?
How do you feel when you are having problems?

    My overall state of my wellness wheel is pretty well. It was about half and half with my assessment. The lowest my average assessment went down to was a two. That was in the social area which is my weakest. My weakest areas are vocational, social, and intellectual.
