Crab Bar Installation: A Case Study
SC contacted Pa PCP and requested Rx for grab bars installation. SC made an ordered OT evaluation from DME provider All Care Medical and several others SC was unsuccessful in obtaining an appt because the DME provider does not have OT or PT on staff. SC called Pa and informed her. SC informed Pa to get another Rx from her PCP to send an OT to her home to complete the evolution and then have the result faxed to SC. SC informed Pa that once SC receives the OT evaluation; SC will complete service order in oracle and faxed referral and script to ALL Care Medical for equipment and installation. Pa reported that she will call her PCP and get things started she is looking forward to getting the grab bars install. 3:47-3:35 on 1/05/16.

    BRIEF HISTORY: This 40-year-old Latin female was admitted for low back and right lower extremity symptoms. She had been suffering from intermittent low back pain dating back to an MVA several years ago. In December of last year she started a job where she was doing a lot of heavy lifting. Shortly after that she began experiencing pain in her back and right leg. CT scan was done that showed questionable disk bulging. This was followed by a myelogram that showed no definite defect. However, repeat CT showed marked compression at the L4-5 level. Some ligamentum flavum prominence and low-grade disk bulging. On May 27, the patient underwent a lumbar diskectomy at L5-S1 on the right side and a disk herniation was identified at the time of surgery. Postoperatively the patient had resolution of her low back pain and right leg symptoms and was doing reasonably well at the time of discharge. She remained afebrile throughout her hospital course. Her incision was healing nicely and she was ambulating without difficulty.…

    On explaining our desire to transfer him using the hoist , he declined , stating that he could do it himself ( he has some degree of short term memory loss ) and he asked if he could be assisted to sit up on the edge of the bed and given his zimmer frame ( this is the procedure we followed up to a couple of months before , when we would assist him to do this and he would then be able to stand and walk independently but it came to the point where he found it very difficult to stand and once standing he was very unstable and not able to walk all the way to his chair or transfer to a wheelchair and as we we were not allowed to manually lift him as it would contravene legislation like manual handling operations regulations 1992 and HSE guidelines that basically mean we can only manually lift babies and small children and ensures the safety of ourselves, our colleagues and the person being moved - so assistance and advice was sought from his named Registered Nurse who made a new risk assessment using the procedures and other moving and handling documentation which is in the library and the manuals that come with the hoists and slings e.g. to make sure he had a sling appropriate for his size and weight , this was agreed upon by him and his daughter that he should be transferred using a hoist from now on ) once he was reminded of all this he agreed that it was OK for me and another carer to move him using the hoist and we explained how we would do it . The care plan is checked again to see which hoist is to be used and which size sling should be used to safely transfer him to a wheelchair.…
