Mrs X has diagnoses of early stage of dementia moved in to our community from her own home, where she was independent and on the assessment family did not mention any concerns.
Mrs X moved in to independent living where on the first day of admission did not show any signs of concerns apart being a bit confused .However on the first night of admission she was wandering form room to room looking for a way out and believed that she is living” just around the corner” not really aware of whole situation She believed she is just in the hotel -night staff on duties reassure her and she went back to bed .After few hours sleep resident packed her own bags and left the building at tis time, she was very confused not knowing where to go and what to do and she began to show signs of verbal aggression.
My role as a manager is to act in the best interest of the resident and I Immediately informed my local safeguarding team , had a conversation with our GP and family -in the family meeting were we did decided to asses her needs again by involving a psychiatric nurse who did a mental capacity assessment.
Resident moved in to Reminiscence department this same day with agreement of family and other professionals ,however reminiscence department is a secure unit where all door are coded and Mrs X felt like in prison, as a manager to make sure that she has her rights I did get the deprivation of liberty department (DOLS) involved where Mrs X was assessed for the mental

The American Code of Ethics for Nurses (ANA) with interpretive statements acts as a guide for professional conduct by outlining the ethical values of the profession. The Code provides the profession with a statement of responsibility to the public and serves as a basis for individual nursing decisions in clinical situations when ethical dilemmas arise. The code integrates universal, ethical principles which serve as the foundation for ethical actions. Nurses face ethical dilemmas on a daily basis. Our primary goal as nurses is the commitment to protect our patients. Provision 2 of the American Nursing Code of Ethics, describes the nurses primary commitment is to the patient. (ANA, 2010). Another important goal is to maintain safety, dignity and accountability. Respect the patients confidentiality and autonomy. The Nurse acts as the person who helps and advocates for the patient and their families about health care and end of life decisions. According to the American Nurse Association Code of Ethics, Provision 11 states that, a fundamental principle that underlines all nursing practice is respect for the inherent worth, dignity, and human rights of every individual (ANA, 2010). In the case study of Marianne a comatose patient, she has no advance directives, leaving her husband and children to make the decisions with different opinions. (Chitty Black, 2011, pg.175). The ethics committee is already working on the case and are assisting the family in the decision making process.     Mrs. X was admitted to low care residential facility in a respite care. Generally Mrs. X was alert, orient and independent with activities of daily living (ADLs). Mrs X actively participated in exercises, knitting, book reading and was mostly attentive on activities organised by the facility. But once during my night shift Mrs. X was found wandering outside her room and was inappropriately dressed. When I approached, Mrs. X started screaming and used abusive language. Mrs. X was constantly asking for breakfast at that time. I reminded and reorientated about date, time and place; reassure her breakfast will be served in the morning and assisted to her to put on the bed. Mrs X’s room had a strong odour of urine, her bed was wet and her cloths were soaked wet. I assisted to make her dry and comfortable. That was the first episode of Mrs. X being confused and being incontinent. On my next shift two days later Mrs. X was feeling weak and unable to transfer herself and mobilise.
