An inventory of all cases currently open, or those closed within the last 45 days, is maintained. Cases are established based upon input made by the Field Finance Offices via DJMS, by file searches and management notices, and created manually by an Employee. The CMS system also provides management with a tool to validate standards, evaluate training needs, obtain accurate statistical data for workload control, and monitor corrective actions taken by Central Site.

Emily Jones attended the CCAC in person and requested assistance in finding Long Term Placement Services for her 94 year old father. A case manager sat with Emily and was informed that the client resides in a rural area of Renfrew County in a large turn of the century farmhouse. The primary source of heat is an external wood stove which requires manual feeding of three foot logs once a day. The electricity is only available in three rooms in the farmhouse; the bathroom, bedroom and kitchen. The client has some small livestock on the farm that are free range feeding. The client has been widowed for two years from the wife that he has had for 74 years. The family’s main concerns are that Mr Jones is not coping well alone in the country, the family members fear for his safety , and feel that he would be better off in a Long Term Care Facility.…

Case management has become the standard method of managing health care delivery systems today. In recent decades, case management has become widespread throughout healthcare areas, professionals, and models in the United States; and it has been extended to a wide range of clients (Park &ump; Huber, 2009). The primary goal of case management is to deliver quality care to patients in the most cost effective approach by managing human and material resources. The focus of this paper is on the concept of case management and how it developed historically, the definition of case management, the components of case management, and how it relates to other nursing care delivery models.…

The scope of social work practice is remarkably wide. Social workers practice not only in the traditional social service agency, but also in elementary schools; in the military; in business, factories, and offices, in federal, state, and local government agencies and legislative bodies; in private practice as individual, family, and marriage therapists; in hospitals and in mental health facilities; in courts and correctional settings; in home health care; and in services to the elderly. In fact social workers can be found anywhere and everywhere there are people who need the help of a professional to alleviate personal or social problems. (Senator Daniel Inouye, D-HI, April 1986)…

In my elementary school, I would include nutrition and health services, integrated case management, and programs to help link the parents and community to the students. We all want students to learn and progress in every area possible; many times, student achievement and growth is hindered by factors before occurring before the students even get to school. When students go to school hungry or not feeling well, learning is most likely not the first thing on their minds. Also, I believe integrated case management is needed because often times, students and families who qualify for and need one service qualify for and need other services as well. These services are often offered by separate agencies, and case management would help to bring some congruence to the students and families and human services delivery. One reason found is that schools operate during work hours, and many parents are unable to leave work for appointments, especially those for whom every penny counts. Implementing programs to help link parents, and even the community, to the children’s education and overall well-being could really help the students improve…
