Adult Home Case Study
On 4/22/2016, CM met with the client to complete Bi-Weekly ILP Review. In the meeting client appears cooperative and friendly. She was dressed with appropriately for the weather. As usual client was quiet in the meeting. Client affect was flat.

SOCIAL SUPPORT UPDATE: Client continues she is a loner and most of the time she spent the day in the dorm reading book. She also mentioned she may socialize with a few residents in the dorm. CM continues to encourage the client to take a walk around the block and to participate in onsite Recreation Program.

OTHER: Client continues to report her new Case Coordinator at Catholic Charity is Shameeka and she meet with her 2x a week at the shelter. EMPLOYMENT UPDATE: Client is unemployed due to medical issues

RESOURCES: Client has no income and CM assisted the client to apply for

HOUSING: Client has no income and 3/30/2016, client applied for PA via Homebound determination its pending. Once client PA is active. Client housing option will be Adult Home or as sited Living Facility.

OTHER: Client psychosocial evaluation was updated and submitted for review. Client is scheduled to meet with onsite psychiatrist on 4/25/2016 at 11:30am. CM issued the client with an appointment slip. Client agreed and signed the appointment slip.

WORKER ASSESSMENT: Client continues to show progress toward independence. She is able to schedule her own appointments. CM encourages the client to take walks around the block. Due to the client medical issues she is a candidate for either Assisted Living or Adult Home.
Plan of action: Client to participate in all scheduled meetings, client must reschedule Access of Ride Assessment appointment, client must maintain medication regimen. CM reviewed Bi-Weekly ILP. Client agreed and signed. Next ILP Review is scheduled for
