Case Study: Aurora Home Care
Pa’s health is declining and has multiple chronic medical conditions and requires constant monitoring and support such as: set up, and hands-on assistance with medication management, bathing, dressing, grooming, transferring, toileting, and meals. Pa is a fall risk due to OA and unsteady gait she uses a walker and cane. Pa informal CG will be unavailable for one week and requesting a temporary increase of 6 hours for total of 12 hours a day from 4/14/2016 Thursday–Wednesday 4/20/2016). To ensures the Pa’s health and safety. The Aide will assist with P.M. ADLs/IADLs and medication management.
Current Care Plan
Agency Model PAS: Aurora Home Care PAS provider 6 hours x 5 days M-F and 4 hours x 2 day Sat & Sun.
