* Number of workers involved in this process. * Number of departments using the sheet. * Number of beds in each department. * Policies of the hospital regarding the sheet. * Who will be responsible to write &signed this document & who will record it in computer system. * Their Qualifications & skills in this issue and their Job titles. * Training programs about this subject, what the percent of the member of Heath care team who had attend & pass it then practice it & also for computer skills. * Resources need (Human e.g. health care providers, physician ,nurse, IT engineer or non-human e.g.format,computer system …) * Materials used e.g. IT system, formats&documents, files.
Process:-
* Write Medical observation & Medical sheet using format & computer system with concerning their order , location in the Medical Record, How it’s written in Name & specified dose for each & frequency/Day with the duration ; also, if there was any significant adverse or allergic reaction and it’s to be signed by the responsible health care provider with daily updating & computer recording.
Consider:-
* How it’s done & written. * Whether it’s done this way or not. * What the percent of those who conform to do it this way. * Is it done in right way or there aremany faults. * Reporting * Frequency of checking the sheet.

Outcome:- Wrong or Medical record errors not ≥ 5% * Number errors detected by departments using the sheet * Number of action taken to correct
