Student Behavioral Observation
Behavioral Observations: Ms. Johnson is an 18-year-old African American female who presented for a vocational assessment on March 07, 2017. Currently, Ms. Johnson is a student. Ms. Johnson arrived on time for her scheduled appointment unaccompanied. Because Ms. Johnson is a transitional student, her means of transportation was the local school bus. She appeared her stated age and was of average height and weight. Also, she was neatly dressed in formal clothing with no apparent body odor. Ms. Johnson‘s orientation and mobility skills were proficient. Her motor activity was normal. Ms. Johnson’s mood and affect were appropriate during the assessment characterized by her alertness, orientation, and thought process. Ms. Johnson related easily

Johnson stated her vocational interests are working with animals (possibility veterinary assistant), helping people (possibility a CNA), or possibility joining the army reserve. Ms. Johnson stated that she is waiting on her results from the ASVAB exam to join the army reserve. Ms. Johnson stated she is willing to work any job expect construction. Ms. Johnson stated her hobbies or other interests are watching TV, eating, playing with her animals, and shopping.
Availability: Ms. Johnson stated that she will accept minimum wages for payment. While in school, she stated that she can work part-time. She is willing to work around her school schedule. She is available for any days and any shifts. She stated that she does not have cultural or religious barriers for employment. She has her driving license and she has her own vehicle for travel to work. She does not have any work experience at this time.
Medical Information: Ms. Johnson denies any significant medical history such as asthma, blackouts, seizures, allegories, or physical restrictions. However, she does wear prescribed nearsighted glasses to improve her vision. She currently receives Medicaid as her health insurance.
