I interviewed Joanna Hawkins. She works in a private organization for dialysis, called Fresenius Medical Care. It is in Birmingham, Alabama. She has been working with this agency for 12 years. The primary tasks she completes everyday are, going around to all the 24 chairs that they have patients at and tell them hello and ask them how they are doing. She will then ask them if they need anything, whether that be need for transportation, need for medications, need for help with their home environment, or need for general support. She works with three different clinics, so she only gets to see the patients at each clinic once a week. When she talks to each patient she must make sure she takes good notes, that way she can help each patient with…

The ER waiting room smelled stale. Sick babies and toddlers were crying, waiting to see a doctor. My brother was laying on the tan tiled hospital floor. He was shaking terribly as the antibiotics from the previous hospital wore off and the infection set in more. Both of his hands were red and swollen. His right hand, had a black mark from where the IV initially infected. His left hand was swollen to the size of a small balloon, kind of like a blown up rubber glove I used to get from the doctor when I was younger. My mom took her jacket off to cover him. His skin was hot to the touch but he was still freezing. His fever was 101.2 but there were no open rooms in the ER at our second attempt at a hospital. We had to get him antibiotics…

Rotation Day 8 today I’m in processing it was a slower day than yesterday so the processer had me help with a few things. They also asked me if I could go with a phlebotomist to drop some blood to the Operation room. When we got there I noticed read lines on the floor meaning we could not pass until we had full gowns. We waited to see if there was someone we could flag since there was no one in the front office. A person came in and we told her we had a delivery she took it and we left. When we got back they had a request to draw a patient in the SSI department I didn’t know where this was so the phlebotomist asked if I wanted to go check it out so I went. I didn’t know it we had to drive since it was in another location. We went in and the patient was in the recovery room she was alert at the moment and recognized the phlebotomist right away.…

As Ms. Smith, a Registered Nurse with ten years experience in the orthopedic unit, entered the floor, she was alarmed to see that one of her coworkers had called in sick. In this 30-bed unit, the typical staff to patient ratio was 1:9 or 1:10, a ratio borne out of necessity due to funding cuts and decreases in Medicare reimbursement, as well as shortages in the pool of nurses. Ms Smith knew right then it was going to be a long stressful night because not only did she have to take care of her normal ten patients she would now have to take on about fifteen, five more than normal. Ms. Smith, after reviewing patient information, walked toward the room of her first patient on this night, an elderly gentleman recovering from a hip replacement earlier that day. Although it was after 11:00 PM, a time when most patients would be sleeping, five of her patients had received prosthetic joints that day and required additional care, particularly related to pain management. While talking with the first patient, Ms. Smith could…

Cathy was stressed and had to plan how she would “attack” the doctors with questions, if she hoped to get any answers. After Al’s time at the hospitals, Al enrolled in a rehabilitation facility, Magee (Bauby, 1997, p. 104). At Magee, Cathy had to adjust to the idea she could easily communicate to the health care providers; Cathy was accustomed to trying to pry information out of the doctors like “guerrilla attacks” (Crimmins, 2001, p. 115). This change that Cathy witnessed is transferable to Mr. Bauby’s experience in acute care. Lack of communication was the “normal” and resulted in a colossal effort to have the option to communicate. Looking at Mr. Bauby’s and Cathy’s experiences, the barriers the staff and doctor are facing is the busyness of their jobs. The staff most likely did not realize that they were not giving Mr. Bauby or Cathy an option to communicate. Although a lack of consideration to be patient and give Mr. Bauby or Cathy the option to communicate led to poor patient care. The staff can adjust the time they give to each patient, asking questions, and waiting for a…
