Well-Groomed Mental Health Case Study
G- CM met with client for a weekly case management, and to continue assisting with his housing goals. I- CM inquired about wearing a protective mask, and his physical health. CM assisted client with updating his medical/housing appointment list. CM praised client for attending all of his housing and medical appointments, but addressed him with being more responsible and accountable for his actions. CM continued to assess client’s mental health and medication compliance. CM inquired about his appointment on 4/6/16 with Laurie Garza from LAMP VASH. CM informed client that Laurie was going to visit the program on 4/14/16, and advised him to speak with her to reschedule his missed appointment (4/13/16).

Client stated that he has been staying active to help decrease his depression and anxiety. Client informed CM that he attends a Korean church in Hollywood, watches movies at the theater, and takes the Metro Rail to different cities (Arcadia, Long Beach, and Santa Monica) to stay active. Client also stated that he feels really positive about receiving his VASH voucher, and is ready to leave Skid Row. Client stated that he felt really good about himself, because he made contact with Brother Richard, from a Christian Parish, and they were going to schedule and set up a BBQ gathering for the program in June 2016.
