Medical: At this time there are no medical concerns. YCS Laurie Haven Staff will transport Jy’Nir to all of his monthly medical appointments (i.e. dentist, pediatrician and eye doctor) when that time comes.

Action: YCS Laurie Haven staff will assist Jy’Nir to all of his scheduled medical appointments.

Education: Jy’Nir will continue to address his educational, therapeutic and clinical needs at Nuview Academy. It was reported by Eva that Jy’Nir continues to be disruptive in school. She reports that his behavior is inconsistent, he is oppositional and refuses to complete his class assignments. Eva reports that Jy’Nir continues to walk out of class and yesterday he receive 7 out of 21 points on his point sheet. CM mention to the team that Jy’Nir behavior in school continues to remain the same. CM suggested that another school placement will be a better fit for Jy’Nir. CFT will discuss possibly exploring a new school placement for youth. …show more content…
Other: Ms. Barnes informs the CFT that she started a new job at Target on Monday.
