Worked on discharge, treatment plans, and connect patients with resources
Observation:
Treatment plans are different within each facility. Practicing some direct service apart of the discharge process.
Subjective:
My site supervisor wanted me to create treatment plans and finish treatment updates on the patients. She thought I knew what to do since constructing treatment plans and writing treatment updates was a part of my responsibilities. I explained to her that the way this hospital does treatment plans and updates are different from the other facility. She was able to guide me through and teach me something new. Connecting discharging clients with services in the community was a good feeling to know we are setting them up with something

    Guess what? I am so excited! I have just been promoted to Program Manager! I am going to make such a difference for this Organization. There will be so much to do you know, with the Programs since they all have a limited time on them. Things like Purpose of Evaluations so that I can review and possibly make it better for the outcome of service/program, Evaluative Research which I can look into research on methods, Descriptive Research will be awesome because I can explain what is in the studying that goes on. The Theory is a great one; it is facts, propositions that are used in the relationship to everything of the program activities. I can oversee the Outputs as well making sure that the outcomes of the services are achieved.     Ms. Copeland is seen for her summary conference from her work up here at Hillcrest Memory Diagnostic Center I initially saw her on April 28, at which time there was a suspicion of depression she since had a CT Scan of the brain with contrast on April 30, which was remarkable. Laboratory studies where completely negative to include normal thyroid function B-12 and RPR. She had a formal neuropsychological battery with Dr. Stella Dickenson on April 26; she scored 136 on the dementia rating scale which is within the normal limits for her age.     I had the opportunity of observing a case being reported. It was reported by an individual who was directly impacted by the occurrence. It was evident that the reporter had reached a point of helplessness. The way she expressed the situation and the emotions that exuded from her showed how frustrated she had become. While listening the report, I started brainstorming solutions and interventions for this individual. I believe this is an area in which growth is evident, I often visualize interventions for the clients that I encounter directly and indirectly.
