Diabetes Evaluation Plan Analysis
The aim of the evaluation plan is the assessment of the process and the outcome of the implementation plan (Fineout-Overholt & Johnson, 2007). The writer implemented the diabetes prevention program based on education and behavioral modification in dietary and physical activity in the high risk individuals in the community church. The implementation program needs to be evaluate the knowledge of the participants of the diabetes prevention program. The evaluation process will assist the writer to find out the effectiveness of the program by examining the change in participant’s behavior before and after attending the diabetes prevention program. It will reflect by their attendance in the educational sessions and their pre and post assessment (see Appendix A).
All the healthcare professionals (volunteer): The outline and the flow of the diabetes prevention program (see Appendix F) presented by the writer to the healthcare professionals was easy to follow. Also they stick to the shared vision and obligated to their responsibilities.
The above mentioned variables for example the participants play an important role in positive or negative outcomes due to their compliance or non-compliance behaviors after the attending information sessions and adaptation of the lifestyle modification. Also the adherence to the outline and the flow of the diabetes prevention program will motivate and encourage the participants and which results in reduction or delay the incidence of T2DM in the high risk individuals of the Community Church.

All the healthcare professional will attend a briefing of the program (see Appendix F). In addition to it, the writer will present an introduction session with the participants which highlights the importance of behavior modification in dietary and physical activity and its positive effects in reducing or delaying incidence T2DM (see Appendix G) and also share the briefing of the program (see Appendix F). By presenting this information with the participants, will encourage and motivate them towards lifestyle modification including dietary and physical activity.

Assessment tool to evaluate project outcome
The comparison of pre and post assessment of the participants (see Appendix A) which includes pre and post fasting blood glucose level, weight, BMI, waist measurement. Post assessment will be done weekly basis for 8 weeks and then at 6 months after the completion of the 8 week diabetes prevention program. Except the fasting blood test which will be done only at the completion of 8th week and then 6th month follow up. The post assessment is valid tool to evaluate the effectiveness of the program.
It is very important to evaluate the implementation program at every level until the program is successfully completed because it will assist in improvement of the ongoing program and provide innovations
