Community Assessment
A community assessment is a process. It is the act of becoming familiar with a community and it explores the health needs of its people and develops a plan to address the necessities (Anderson and McFarlane, 2011).

An assessment was directed and data collected by students of the Daemen College Department of Nursing in collaboration with the Genesis House. The Genesis House is a homeless shelter that serves families, single women and children who find themselves with nowhere to turn during unsettling times. It is located in Olean, New York, which is in Cattaraugus County. The definitive goal of the project is to increase awareness about Hepatitis C in the community, increase risk assessment to aid in prevention, increase testing of high risk populations, providing continuity of care, and improving the overall health of patients with Hepatitis C, who are past/present guests of the Genesis House.

The assessment includes a background of the affiliating agency and focused area of the health objectives of the community. The community assessment will provide a definition of the community, information on the affiliating agency, approaches used to assess the community, and a description of the survey results identifying health needs, risks and preventions of a well-defined population. In addition, the community project will include the assessments of the past/present guests of the Genesis House, history, demographics, and subsystems of the community. The community assessment will investigate a health concern among past/present guests of the Genesis House, who presented with the disease, Hepatitis C, while residing in the facility.
