Castaneda Case Summary
On Friday, 1-15-2016, I, Investigator Torres, met with Human Resource Generalist Ms. Marisol Castaneda at Yamaha Motors Corporation at 6555 Katella Ave., Cypress, CA 90630. After a brief discussion, she agreed to provide a recorded statement. The details of the recorded statement are as follows:

Ms. Castaneda has been the Human Resource Generalist for Yamaha Motors Corporation for approximately ten years.

On 11-24-2015, Ms. Castaneda stated she conducted a follow up on an email she received from Supervisor; Mr. Lou Nakawawa in the Business Analyst Department. The email contained information regarding a discussion with an employee named Ms. Sharon Lie, whom Mr. Nakawawa supervises. Within the email, Mr. Nakawawa stated that the claimant was inquiring about having her workstation ergonomically designed to suit her needs for the pain she was experiencing in her bilateral wrists, elbows, arms, shoulders, and neck. Castaneda stated when she followed-up with the claimant on 12-24-15 the claimant told her that she developed CTS to her bilateral hands, wrists, elbows, and shoulders, which now has traveled up to her neck and upper and lower back area. At no time, did the claimant ever mention that her CTS injuries were from a previous claim that she filed with another employer.

Ms. Castaneda had stated before she completed a claim for the claimant; the claimant completed a Reasonable Accommodation Request for the ergonomic workstation. She also completed an Employer’s First Report of Occupational Injury or Illness Report. Ms. Castaneda learned from the Employer’s First Report that the claimant had noted that her initial injury occurred on 9-5-2012 that was her first day at work when she was hired for her job at Yamaha Motor Corpo. .

As of yet, Ms. Castaneda has not authorized the claimant to be seen by a medical clinic and is waiting for
