Basak Sickness Case Study
I know you are busy, but I would like to tell you about the beginning of Basak’s sickness. Basak used to like ice cream, and she used to have some of it as a daily basis. One day, she said that she would not have ice cream by explaining that ice cream blocked in her stomach. She began not to have ice cream anymore, and she began to suffer from constipation and bloating in her stomach when eating even small size of food. Then, we took her to a regular doctor at Med7 in Sacramento, CA, and the doctor said she was just OK. I asked the doctor to check her entirely, but the doctor didn’t do it, and I didn’t insist on it because she was pregnant. But, Basak had some pain and sufferings from her stomach, and we took her to the ER at Kaiser Hospital …show more content…
The ER doctor didn’t take care of her, although Basak severely suffered from her stomach, and had pain and swelling. The ER doctor didn’t take care of her complaints, and she just focused on Basak’s weight. Then, the ER doctor sent her to UC Davis Children hospital with me. In the Children hospital, Basak was not able to eat well enough because of her stomach swelling. She couldn’t even bend over, and I helped her for 12 days. Then, she continued to lose weight in the hospital. The hospital was just interested in the diagnose as anorexia. They didn’t care about her stomach situation, and they wanted to send her a facility where anorexia is treated. I objected them and asked for a treatment and check in the hospital. Then, they saw some kind of stomach largeness in the X-rays, and they thought that the stomach was getting larger. Day by day, her stomach began to get larger, and she was not able to pass the stool. I talked to a nurse, and I suggested her to give a laxative pill for Basak’s constipation, and the nurse listened to me, and gave some pills, and then Basak had very large stool, and got relaxed. Then, she began to eat well

AG is 50yo female with a history of Sclerosing Mesenteritits, diabetes, hypothyroidism, smoking and alcohol abuse. She was admitted on 1-27-12 for severe abdominal pain along with nausea and vomiting. CT scan of the abdomen revealed a massive gastric dilation and thickening of the duodenal wall leading to occlusion of the duodenal lumen. I met AG on 2-23-12 when she was three weeks post-operative (post-op), gastrojejunostomy, resection of the pancreatic head mass biopsy, and gastrostomy tube (gtube) placement. The patient had her first episode of acute pancreatitis in 2006 with four recurrences over two years. In 2007 she underwent Laparotomy and had a biopsy of an inflammatory periduodenal mass. She was treated with radiation and chemotherapy for a short period. In 2008 following several episodes of abdominal pain she underwent endoscopic retrograde cholangiopancreatography and had placement of a biliary endoprosthesis. In 2009 she had placement of a feeding jejuostomy for duodenal obstruction. She then underwent the Frey Lateral Pancreaticojejunostomy with resection of the Pancreatic head and Hepaticojejunostomy with Roux-en-Y reconstruction and small bowel resection in 2010. The procedure was complicated by liver abscess requiring percutaneous drainage. Diabetes was diagnosed after her pancreatic surgery in 2010. She is currently on an insulin sliding scale.…

I had some concerns for this patient’s safety so I asked him if he was taking medication for depression. He stated he was. I inquired if he was seeing a psychiatrist for his condition. He replied,” I have not seen him in a while”. We discussed whether he should make an appointment with his doctor soon. I asked him if one of our dialysis doctors could talk to him about what was going on because I was worried for his safety. The patient needed to know that since he wanted to harm himself I had to document this in his chart and let the doctor know. He nodded his head up and down. I also informed the patient that someone would be contacting an interpreter so the doctor could communicate with him.…

My doctor told me that because I’m young and active so it very normal to experienced it. So now I’m siting in the ER thinking how could it be, I had my check up a week ago and everything is fine. After they did a complete metabolic panel and Hemoglobin A1C, the result revealed that my blood glucose at the time was 300 and my HA1C is 12 %. While I was rehydrated with IV fluid, the doctor also orders a potassium rider, because my potassium was low. Long story short, there was a miscommunication during change of shift report; I was given more potassium then my body required.
