Subject: Statement Report of Accident in the Computer Labs

How the Accident Occurred
At, 1000 hours on Tuesday, 5 February, 2013, during the CSI 142 labs classes in Block 247, room 293, a student by the name of Bosa Matlhare – while working on his java exercise brushed his elbow against the power cord of the computer while attempting to fix the Local Area Network cable on the computer that was assigned to him for that lesson. Not having observed that the power cord was loose, the electrical surge caused a shock in him, causing him to panic and bump into the monitor of the computer beside him – which we only found later had loose wires – with enough force to knock it off balance and on to the fellow student beside him, Katlego Olefhile, injury her arm as she attempted to shield herself from the monitor. She was left with a fracture in her forearm and a swollen elbow, resulting from the bump onto the table while attempting to shield herself from the monitor.
What Actions Were Taken
The lecturer – having not seen the events that taken place – immediately resolved to reprimand Mr Matlhare, but with the aid of Mr Kiilu Jr. and Mr Rampa, the events of the story were able to be unfolded and Ms Olefhile was rushed to the UB Clinic, where Mrs Mogwe the nurse on duty attended to her with quick precision. She was given a cast and her swollen elbow attended to professionally. The loose wires have been replaced with new ones and the computer technicians inserted placeholders to place the wires firmly in place to avoid them moving around and getting loose. Ms Matlhare agreed not to press charges, being quoted on record as saying, “I understand it was an accident, and I am grateful to Bosa’s quick assistance at helping me.”
Precautions Taken to Avoid Future Accidents Teacher Assistants were advised to no longer allow students to meddle with the wires, as it was suspected that the wires had been made loose due to
