Chapter 1

background of the study
Three hundred sixty six (366) million people worldwide suffered from Diabetes Mellitus (DM) last 2011 and will rapidly increase through the following years. 4.6 million of them died and 183 million people (50%) remain undiagnosed. Even children (78,000) are affected by this disease (International Diabetes Federation, 2011). These rates show that DM is an alarming and fatal disease throughout the world and the fact that it does not choose whatever age, gender, race or type of people there are is terrifying. Diabetes Mellitus is a metabolic disease that is characterized by an increase in normal plasma glucose level resulting from imbalance of hormones, insulin secretion defects, insulin action or all. It is the 9th out of 86 major cause of death in the whole world ( World Life Expectancy, 2010) and 6th to the Philippines causing 18, 512 deaths in the country ( World Health Rankings, 2010). There are two categories of DM: type 1 (Insulin-dependent diabetes mellitus) and type 2 (non-insulin-dependent diabetes mellitus). Type 1 diabetes is characterized by hyperglycemia due to the destruction of pancreatic islet of langerhans and cellular mediated autoimmune destruction of β-cells. It is also known for its absolute insulin deficiency. Type 1 diabetes results from the genetic predisposition of the person and may have nephropathy, neuropathy, retinopathy and heart disease. Type 2 diabetes is also characterized by hyperglycemia due to insulin resistance plus an insulin secretory defect. Relative insulin deficiency is the result of the resistance. Diabetic people who are at elevated risk for various serious health complications, including cardiovascular disease, premature death, blindness, kidney failure, amputations, fractures, frailty, depression, and cognitive decline. Different treatments are needed to prevent further complications. These are available in drugstores but because of poverty in some countries, like the Philippines, DM remains unattended. A natural and non-depletable source could be a good remedy. There are several reasons why medicinal plants should be studied: (a) to recognize therapeutic effect (b) and its side effect. Guyabano (Annona muricata Linn) is a plant that can easily be found in tropical countries such as Philippines. The Guyabano tree has its upturned limbs that’s why it is low-branching and bushy but slender, and has a height of 25 or 30 ft. The stench leaves, normally evergreen, are alternate, smooth, glossy, lighter beneath the surface, and dark green on the top; oblong, or elliptic, pointed at both ends. The length 2 1/2 to 8 and the width is 1 to 2 ½. Young branchlets are rusty-hairy. The flowers, which are singly when borne, may appear anywhere on the trunk, branches or twigs. They are short stalked, 1 1/2 to 2 in long, round, and triangular-conical, the 3 fleshy outer petals are yellow-green and the 3 inner petals are pale-yellow. Aside from an anti-cancer property, anti-inflammatory, antibacterial property, researchers say that the fruit has an essential effect in lowering the blood glucose level. Objectives
