LATE ENTRY

On 8/6/2016, CM met with the client to complete Bi-Weekly ILP Review and housing update. Client was dressed with proper attire for the weather. She appears to be worn out and tired. CM inquires the reason client appears to be tired. Client replies; “She is up early every day for work at 2am to leave the shelter by 3am.” Client continues to report she also suffers from sleep apnea and she will like to be housed in the Bronx. She continues to report she work in NJ and there is a van that picked up NY workers and transport them to NJ, and client continues to report she will prefer to be housed in the Bronx.

SOCIAL UPDATE: Client reported she does have family members here in NYC, but she cannot reside with them, because her parents live in Section 8 housing and they suffer

She also reported she did a sleep apnea test and she is unable to pick up the result because she doesn’t want to miss days from work. CM advises the client to pick another PCP that work on Saturday. Client reports no changes in her medications.

SUBSTANCE ABUSE: Client continues to reports no past or present substance history

MENTAL HEALTH: Client continues to reports no mental issues.
HOUSING UPDATE: Client is a GP housing package is circulation. She also has an update Linc V certificate. Client this week didn’t submit any housing log.
CM advises the client to view at least three apartments per week and submit proof of apartment search log. Client mentioned she is having difficulty with the voucher because most broker and landlord are not accepting the voucher.
Client brought the following low income application for CM to fill out. Webster Commons E, 87-14 116th Street, Richmond Hill, NY 11418 and Crossroads II Plaza, 87-14 116th Street, Richmond Hill, NY 11418, 1770 TPT LLC 1394
