Schizophrenic Case Study
Primary diagnosis: Schizophrenic, paranoid and other functional psychotic disorders.

Secondary diagnosis: None established.

The claimant was a 47-year-old woman.
Alleged disability: schizoaffective-bipolar, joint and back problems that cause severe pain, congestive heart failure (CHF) and renal failure in 2007.
She reported that she spent four hours daily at an adult day care center. She used a roller walker when she felt weak, secondary to (CHF) and renal failure (2007). She had difficulty with mobility, all range of motion and leg movement caused pain. She struggled with activities of daily living and lifting objects. She also had trouble with memory and concentration, and completing tasks. She had problems sleeping (napped during the day), was claustrophobic, needed reminders related to her personal care, and could not handle money; she gave it away

She helped with the care of her dogs. She had some home help who did most on the inside chores (Medicaid waiver). She also had support from others who helped her with her children.

The Comparison Point Decision (CPD) date was 01/23/1995.
Electronic files related to the CPD determination were not found in the electronic records. Per the “Status/History” section in the electronic records, there is a paper folder containing medical evidence for the last favorable disability determination.
Per the “CDR Analysis Form” (12/23/2015), the claimant had a history of drug overdose, polysubstance abuse, depression, and bipolar disorder.
