Re-Collaboration With Nurses In Home Care
Jennifer, I also provide teaching to my patients and their families since the first day of admission. This process is continuous and day-by-day they learn more about the skills, starting with simple things. Sometimes, when the procedure is very difficult for them to do, home health companies continue to care of patients, and nurses from home health come to our rehab facility to access patients and ask us how to provide specific wound treatments. Some of patients are panicking in the beginning, because they do not know how to perform the treatments at home. Nurses in rehab with nurses from home health companies work together to minimize patient's anxiety level, teach patient with his/her family and reduce the chance of re-hospitalization. Thank

    There are many advantages to partnership working. The main advantage being, that when all partnerships work together it enables the service user to have a better quality of life and sustain authority and freedom over their own lives. Good Partnership working between individuals and health and social care providers will encourage compliance with care as well as promote a positive outlook with a service which people participate in rather than simply receive. Each Health and Social Care provider will have their own expertise in their own line of work. They will also have their own unique relationship with the individual, here are just some for example?     In order to gain Mary's trust and reliability, as a manager of the care home I have to advice Mary's social worker to offer her the affection and support she has never received, in the care home. In order to effectively inform the social worker about the details of Mary's situation- a very thorough verbal discussion will have to take place.
