This is 34 year old Oriantal male. Patient is here for his medication refill and labs. Patient was seen at UAB ED 2 weeks ago related a fall secandary to posible siezer activity. No abnormaity reported form UAB. Patientis also flowed bya pschyatric departement the visit aws one week ago. no medication changes at that time. patient is a cureent smoker with 25 year pack history.
