Great Grandmother Incident
EREPORT # 19495 stated the following: The incident happened on 3/11/2016.
Maternal Great Grandmother (Mattie Triplett)has been told numerous times to stop feeding child, R’Mon Ware , by mouth due to the possibility of aspirating . Child has a G-Tube. Great Grandmother, Mattie Triplett constantly puts food in child’s mouth. Ms Triplett feeds him several solid meals a day against medical advice. Child is unable to sit up on his own, hold his head up on his own and is severely developmentally delayed. Ms Triplett has been told several times not to give R’Mon solid foods but tells medical staff that she knows what she is doing and has raised (X) number of children. Biological mother (Samantha Triplett) of R’mon has since had another child, a newborn, who is dropped off at the same Maternal Greatgrandmother’s

Child has frequent seizures, is always on his back and greatgrandmother is unable to safely carry child around the house or to and from every medical visit due to her own limited mobility and medical issues. She is unable to ambulate safely with the newborn baby either due to her fall risk. The Great grandmother also has 2 other biological children of Samantha Triplett’s who are physically abusive to the greatgrandmother, they sneak out of the windows in the middle of the night and have serious educational challenges (both children have been retained previously) . I am R'Mon's former PHRM/ISS Case Worker. I was notified of certain information by a family member, and observed certain things while providing services. A patient advocate contacted me recently in reference to the G-tube. Description continued: Biological mother is completely aware of these circumstances but still leaves her newborn and R’Mon (her disabled child) as well as their 2 older siblings with her grandmother inspite of Ms Triplett’s inability to care for them.     On Thursday September 14th at around 12:30 pm the hospital had a sentinel event which involved a missing child. The event details are as follows: Tina was scheduled for same day surgery and was brought to the hospital by her Mother. Her Mother was informed that the surgery would take approximately 45 minutes and that Tina would be in recovery at least 1 hour. Tina’s Mother informed the pre-op nurse that she had an errand to run involving Tina’s older sibling. Tina’s mother left the pre-op nurse her cell phone number with instructions to call her if Tina got out of surgery and was ready to go before she returned. Prior to Tina’s mother returning Tina’s father showed up to check on her, and finding that she was ready to go he took her to his house until the mother could pick her up. When Tina’s mother returned 2 ½ hours later she found Tina missing and the staff did not know that her father took her. The hospital activated a Code Pink and notified the local police department. When the hospital security interview the mother they learned the parents were divorced and that she had full custody of Tina and her other siblings.
