Flywithus Airlines Case Study
Scenario 1

FlyWithUs Airlines has started a new low-cost carrier service to link major cities such as New York City with small towns. A few of the airports that service FlyWithUs are located in remote areas and are ill-equipped to handle emergencies.

The airline also has a charter service that flies to locations around the world. In some remote areas, where the airports are small, help may not be immediately available in the event of an accident or some other crisis.

Due to a failure of the air traffic control information system, a FlyWithUs pilot is forced to make a manual emergency landing. The plane skids off the runway and finally comes to a halt in the wilderness lining it. Five people are hurt, and one woman is critically injured. The airport does not have an

Could FlyWithUs have prevented this situation? If yes, what measures could they have taken? Could they have stationed their own ambulance at the airport to handle emergencies? Should their pilots have been better trained to make emergency landings? Select one of the three case studies discussed at the end of Module 6 and respond to the reflection questions.

Scenario 2

The U.S. Army has chartered a flight to Afghanistan in order to transport soldiers needed because of a recent rise in attacks by the Taliban. At a stopover point, FlyWithUs discovers that refueling is not possible because fuel supply lines have been cut due to rebel activity. The team and the plane are now stranded and are waiting for help.

Question: How could information systems have been used to prevent this from happening? What types of security measures are now needed to secure the airplane, its passengers, and its cargo in this dangerous situation? Is there any way in which FlyWithUs could have ensured a fuel supply? Could the airline have arranged to carry extra fuel?

    The sky was clear on the morning of June 30, 1956 when two planes, the TWA Super Constellation and the United DC-7, departed Los Angeles International Airport within three minutes of each other. Both aircrafts had “been flying in uncontrolled airspace, under visual flight rules without the guidance of any air traffic controllers, radar or even official flight plans” (Chang). Both pilots had requested to fly in undesignated airspace thus taking responsibility for the safety of themselves and those on board. The air carriers “drew slowly together in perfect visibility, being apparently within visual range of one another for many minutes; but the TWA DC-7 was below and in front of the United Constellation, and neither pilot could see the other aircraft…there were no survivors”(Machol). The crash killed all 128 occupants aboard the two planes.     On November nineteen, nineteen hundred and ninety six a United Express Beechcraft nineteen hundred took off from Chicago. After an intermediate stop at Burlington, Iowa the aircraft took off again for Quincy, Illinois at four forty PM. As the Beechcraft was approaching Quincy, two aircraft were ready for departure. Beechcraft King Air and Piper Cherokee were taxiing to runway zero four. Since Quincy is an uncontrolled airport, all three aircraft used a common traffic advisory frequency (CTAF). As the United Express Beechcraft was getting ready to land on runway thirteen, it was under the impression that the King Air was holding short on runway zero four, and continued their approach. The King Air however, had taxied onto the runway and had started it’s take-off roll when the Beech nineteen hundred touched down. A collision took place at the runway zero four and thirteen intersection. It was at least twelve minutes before firefighters arrived. The airport has no control tower and no aircraft rescue and firefighting (ARFF) crew. It relies on the Quincy Municipal Fire Department, which had to drive eleven miles. When they finally got there, it was too late, both aircraft caught fire immediately with fourteen fatalities reported.…

    On Friday July 6th 1979, villager and holidaymakers had a miraculous escape when an out of control RAF fighter jet ploughed into the village just yards away from a loaded petrol tanker. The Plane Crash 1979The pilot, who was on a training flight, experienced control difficulties and had to eject into the sea leaving his Hawker Hunter jet to crash into the ocean. Instead it veered towards land, hurtling at 250mph, a major disaster seemed inevitable. But it was nothing short of a miracle when the jet ended up wedged in a 12' gap between Klymiarven Guest house and King Arthur's Terrace, damaging just three cars, a greenhouse and a swimming pool in it's wake. Guesthouse owners, Jim White and John and Muriel Jewell, said they were lucky to be alive, along with Ivan Irons, who was up a ladder painting the first house to be hit, Westmead in Atlantic Road. Garage owner Jack Cann, was preparing to receive1,500 gallons of petrol when the pieces of the jet landed only 10 yards away. If the jet had exploded the whole village would have gone up. If the jet had not been stopped by the houses it could have ploughed down the high street killing many holidaymakers. In this amazing incident no one was seriously injured and even the pilot survived after being picked up by a Port Isaac fishing boat, the Lady Jane. At the time the plane crashed I was on the phone speaking to Fred Whiting, who at that time owned the launderette in Tintagel. I remember we were speaking about a TV repair when he suddenly said "I've got to hang up the phone...a plane is coming straight at the house!". In fact it ended up a few yards away from his house as it veered across a field before hitting Klymiarven and King Arthur's Terrace. I quickly gathered up a few movie films, grabbed my camera and dashed down to the scene. I shot quite a lot of film at the time and later showed it at the Social Hall in Tintagel at a special thanksgiving night held by the Parish Council. It really was an incredible escape from…
