Subjective:
“ I noticed a significant delay in my bowel and I don’t know why?”
According to her this is not her usual characteristic of bowel
Objective:
Defecates 2 times a week
Brownish color of feces, scanty amount and hard
Seldom drinks water
Client has poor eating habits
Change in usual foods or eating patterns.
Urine has scanty amounts and yellowish in color

Constipation related to little amount of fiber diet and inadequate water intake manifested by dry and hard stool.

Experiencing constipation due to sudden change in eating pattern with little amount of fiber diet and inadequate water intake . Low fiber diet results in low fecal residues, which can reduce the frequency of bowel action that is evidenced by dry and hard stool
Short term :
After 4 hours of nursing intervention the patients stool will be softened through increase in fluid intake and eating foods that are rich in fiber.

Long term:
After 5 days the patient will return to normal pattern of bowel movement . With normal characteristic of stool.

Independent:
Determine the pattern of defecation for clients and train clients to do so.
Set the time is right for clients such as defecation after meals.
Provide coverage of nutritional fiber according to the indication.
Give fluids if not contraindicated 2-3 liters per day
Assess usual pattern of elimination; compare with present pattern. Include size , frequency color and quality

Encourage daily fluid intake of 2,000-3000cc per day, if not contraindicated medically.

Collaborative:

Provide coverage of nutritional fiber according to the indication.

Give fluids if both contraindi-cated 2-3 liters per day.

High fiber nutrition to launch
