I am doing my field at the hospice house in Greenville, NC, and then it is essential for us to carry out the HIPPA confidentiality policy. We do not leave mail face-up that is being sent out to former family members. According to policy their information is still kept private. I have a patient, and I carry her information with at all times. However; the name of the person is hidden. Social workers must cultivate leadership in this critical field of practice; the educational programs stress both direct practice abilities and initiative in transforming organizations, service conveyance frameworks, and polices. Therefore; social worker must practice confidentiality according to the organizational rules and regulations to enhance support for people

Identifiers are small pieces of personal information by which a person can be recognized such as their name, social security number, date of birth, email address, account numbers, medical record number, and facial photographs. Before any information can be used or disclosed without the direct authorization of the patient, all personal identifiers outlined by HIPAA must be removed from all documentation. Once personal identifiers are removed the information is considered de-identified health information (McWay, 2008).
