SC, Jennifer Stoker contacted the provider, Kwanisha Moore via telephone. SC introduced herself and told provider that she was Christian’s SC. SC asked how Christian health. The provider noted Christian has had an ear infection , which stop him for going to school for several days. Kwainsha noted he went to the doctor for treatment. Kwanish noted the give medication for the infection. Kwainsha noted his infection has clear up and has not had any more health issue. Progress noted made because Christian was not health
