Case Study: Tina Walters
2/8/2016: I called Tina Walters and she informed me that her brother, Donald Sloan was discharged from Brookwood Medical Center in Birmingham, AL. She stated that his medications were all changed, he is now on Celexa, Propranolol, his Vistaril has been increased and Risperdal. Brookwood Medical Center prescribed a one month prescription with one refill and recommended that the patient follow-up with Mountain Lake Behavioral Healthcare. Tina Walters stated that since the patient is established in Decatur’s Mental Health Center she will keep his counseling appointment on 2/12/2016 and his psychiatrist appointment on 2/23/2016 and request one month prescriptions with two refills to bridge her brothers care until he is able to get established
