What Augustus is saying is that Hazel never noticed his cancer, what she did do was fall in love with him no matter the circumstances. She payed attention to him regardless his prosthetic leg. Another way of seeing this quote in the book is Peter Van Houton noticed that Augustus waters and Hazel Grace did love each other and wanted to pay it to Augustus to attending his funeral.
