DAILY ACTIVITIES

The claimant starts his day between 10:30 AM and 11:00 AM. Throughout the day, he watches television and talk to his friends that are in the halfway house. He goes to bed at 11:00 PM. He talks to his mother on the phone everyday. He has a few friends at the halfway house that he hangs out with. Other people at the halfway house are responsible for the household chores. He refuses to complete his chores. He reports that his cleaning abilities are limited by his back issues. He is unable to cook and needs assistance with shopping.

The claimant wore baggy sweats, a T-shirt and a hooded sweat-jacket to his appointment today. He uses excessive profanity. He can be described as fidgety.

He is very hostile towards other students and coworkers. He was terminated from employment, two years ago, due to aggressive behavior and poor attendance. He has maintained unsatisfactory interpersonal relationships. His mother and stepfather hit him with objects, while growing up. He has no interest in marriage or in having children. He has no patience for people, mistrusts them and anticipates conflict with others, which impacts his relationships. The claimant is impaired in interpersonal functioning. Describe the claimant’s abilities and limitations in responding appropriately to pressures in a work setting.

The claimant expressed work related difficulties regarding stress tolerance. When he would have a bad day at work, he would become angry, upset and act out. He would leave or take a break, as well. After work, he would quickly leave and abuse substances. He reports that he has been involved with counseling since the age of seven. He reports 170 attempts at suicide and cutting self. He was admitted into the hospital for psychiatric reasons two times. He reports that he has been on many psychiatric medications, in the last 5 years. The claimant is impaired in stress tolerance.
