Subject: Progress report (from January 22 – April 26, 2013): design development of deep vein thrombosis (DVT) prevention device.
Introduction
The development of this device is basically to wrap around the forearm of the patient and it will provide accumulated pressure around the forearm. This device is non-pneumatic thus it will not use any air pressure or pump to operate. Current device use in the market of biomedical department is intermittent pneumatic compression (IPC) device, it use air pressure to compresses the limbs. Theoretically, an ideal compression device will compress sequentially (distal to proximal) and will have a pressure gradient (highest pressure distal and lowest pressure proximal). This is where an appropriate solution of using non-pneumatic device will wrap and tighten around the patient forearm due to the rotation of few wheels. Each of the wheels will be holding by its own strap as it will be connected together. Noted here each wheel will have totally difference circumference and will create different pressure.
Work completed from 22 January to 26 April 2013
Up until this date, there are few progresses I have done for this project. Firstly, designing of the device is completed and the design is for the segmentation section that use as a gearing system for the device. Followed by the basic calculation on how to get the exact rotational ratio for the gearing system. Other than that, costing plan and also selection for hardware is been finalized.
Work to be complete
Next, the device needs to be in prototype so that the testing and analyze of the device can be done. A final calculation also need be done so that a suitable motor can be applied.

Problem encountered
Greatest problem that I face for this project is the limitation in term of source of references. Other than that, to get a suitable motor that use as a rotational supply for this device

    Background and purpose: Venous thromboembolism (VTE) is a disease that includes both deep vein thrombosis (DVT) and pulmonary embolism (PE). It is a common, lethal disorder that affects both hospitalized and nonhospitalized patients, if overlooked, can lead to long-term complications. VTE results from a combination of hereditary and acquired risk factors. Vessel wall damage, venous stasis, and increased activation of clotting factors also known as the Virchow triad is the most common cause of VTE mainly DVT. According to the National Institute of Health on the consensus of prevention of pulmonary embolism, all hospitalized patients are to be stratified into low, moderate, high and highest risk levels and treated according to their level of risk. Surgical patients, especially hip and knee patients are classified as highest risk while patients under the age of 40 with minor surgeries lasting less than 30 minutes are considered low risk patients. The preferred form of prevention of VTE in the low risk group is early active mobilization while a combination of pharmacological and mechanical management is recommended for the highest risk group. In this review, we will focus on the effectiveness of Low Molecular Weight Heparin (LMWH) compared to pneumatic compression devices in the prevention of VTE in very high risk hip and knee surgical patients.…

    For a patient who presents without any risks, it would be of the essence to initiate DVT prophylaxis using compression stockings as they “increase blood velocity, promote venous return, and have been shown to be effective” (Meetoo, 2013, p.393). Compressions stocking as mechanical prophylaxis have many advantages, such as: non-invasive, painless, and less costly. In addition to that, patients more than likely would feel mentally better as they are not “getting one more medication”.
