Executive Summary: The Injury Of Lumbar
DOI: 2/13/2004. The patient is a 66-year-old male bartender/food server who sustained a work-related injury when he slipped and fell as he was walking through the dish area.

As per OMNI, the patient was diagnosed with lumbar degenerative disc disease, chronic pain syndrome, lumbar spondylosis and bilateral knee degenerative arthritis.

As per OMNI, the patient is status post anterior and posterior C4-5 and C5-6 fusion on 12/30/05. According to the AME report on 1/30/07, allowance to see the treating physician for flare-ups of symptoms or for medication refills are reasonable. As per progress notes on 2/22/16, the patient rates his pain level as 9 without pain medication and as 5 with pain medication. The patient is currently not working.
