Self-Reflection on personal practice
While on placement, I completed three tasks directly linked with the care of the patients. During the task I was observed by the nurse Amy who supervised me in doing the task. The three tasks involved where;
• To assist a patient with eating a snack / meal
• To transfer a patient from bad to wheelchair and transfer him to his Physio
• To assist a patient with washing their hands before a snack or a meal.
During some of these tasks I felt really anxious even though I have had training prior to assisting. I felt that I could make a mistake in assisting the patient. I was thinking if I was doing everything right as to what I was thought. Some of the patients had challenging behaviour which made me unsure on how to communicate with him, however maintaining a good relationship with the client was very important.
In transferring john from the bad to the wheelchair, I was very confident in assisting him. John’s leg was amputated. The good thing about this was that I was able to reflect to how a relation of mine was in the same situation. I felt that communication with john went very well and I was able to empathise with him, and regained his trust. My practical skills became useful at this stage due to manual handling and patient handling. I was also able to use all the skills I gained in helping my relation. This task went well and I was happy on overall how I did task.
In the second task I was assisting Alan with eating, however this task made me feel nervous as I had to watch his every move as he had swallowing difficulties. The bad thing about doing this task was that I was afraid that food might get caught in his throat, and that I will have to use my train to dislodge the food. This made feel uncomfortable and anxious until Alan finished his meal however I ensured that I fed him cautiously and that the food was at the right consistency. I think I could have than better on this task as to making sure the patient did not

Explain if the goal was met or not met and include rationale for your explanation. The goal was partially met. I had interacted with the patient briefly on a number of occasions in the dining room through small talk prior to our talk. The patient was more apprehensive to keep the conversation going and withdrew due to fatigue. 2. Which phases of a relationship were demonstrated? The orientation phase was completed. Ms. A abruptly ended the conversation during the working phase of therapeutic communication. The subject matter became very close and personal on subject matter to which she presented in the emergency unit. 3. After leaning about each concept in Nursing 201 I have become more competent than before about the profession of nursing in areas such as evidence based practice, patient centered care and many other areas. I think after learning about patient centered care and the nurse-patient communication I am better prepared to interact with patients. Some things that I find to be especially helpful are to not ask close-ended questions or anything that will result in a simple yes or no, use general leads, use silence and to paraphrase and reflect what the patient says.…

    After I felt I had set a calm and relaxed surroundings, I then asked the patient what the problem was and how I could help them, from that I found out that the patient was suffering from severe abdominal pains, it was then my duty to ask the teenager a few questions which could then give me an idea of the possible outcome’s to my patients problem, one of the questions I also asked the patient was if she was comfortable where she was sat or if she would like to lie down the stretch out the pain in her abdomen. I feel I played my role well as a nurse to make the patient feel confident and comfortable, whilst I was asking my patient questions I feel I made good eye contact and a calm tone of voice I also used appropriate language and asked specific modified questions. As a young adult my father suffered two separate heart attacks and had cataracts in both eyes. During this time period, I spent countless hours at Loma Linda hospital shadowing nurses and other medical professionals. I don’t think I took classes at Grand Canyon for the date shown above. I was enrolled there for only 2 systems, but I can try and contact them for my transcript. Please extend my gratitude to the Myer’s Family of how lucky my brother and I are to have such wonderful, caring, and patient people in our lives. I certainly don’t take them for granted, and we apologies for missing the brunch this year. We will try and make it up to them by achieving higher grades this year, and also inform them that I passed the HESI A2 Test yesterday. I will be meeting with a counselor this coming Monday to proceed with the application process to the nursing program at Phoenix College. The wait list is about a semester or two long I heard, and I have also…

    I am currently studying HNC Health Care and as part of my course I have to complete a graded unit, this will entail three stages; Planning, development and evaluation. This will be carried out while on placement within a hospital setting, within the planning stage I have to choose a patient and assist them with a nursing activity. I have chosen to follow Roper Logan and Tierney twelve activities of daily living the reason for this is that I find it to be the nursing model that is most effective as each activity has its own importance to the survival of life; also it follows the objectives of my graded unit. I have chosen the AL of eating and drinking as the patient I have chosen has severe Rheumatoid arthritis and has lost the use of her hands so she is unable to feed herself so my aim is to assist her whit this AL ”For a variety of reasons a patient may require assistance with the actual activity of eating and drinking, and this should be given graciously and with dexterity so that he will be protected from the needless embarrassment resulting from the lack of independence in this AL”(Roper Logan Tierney.p141). While I am assisting with this task I must remember to respect Mrs A’s dignity at all times.…
