Practicum Imperfections
Today it was another busy day. For the first 2 hours of the shift, we had walk ins. I saw altogether 7 patient independently. It is hard to believe that I have only 2 more week left to finish my practicum hours. I started as a naïve nurse practitioner student and now I find myself more confident in collecting history, doing physical exam, ordering diagnostic testing, differential diagnosis and creating plan. It has been wonderful learning opportunity for me with a great preceptor and working environment. The reason for the visits were hives, possible sinus infection, HTN and hyperlipidemia follow up, swollen foot, lower leg pain, lower back pain, stomach pain and crohn’s disease. Patient with hives was given prednisone for 6 days at urgent

The morning of the first day of my clinical preceptorship I had such a mix of emotions. I had been lying in bed awake for hours because I felt too much excitement to sleep. My alarm went off and the nerves set in sending one question after another swirling through my head. Would my preceptor be nice? Would I disappoint her? Would I remember my lab values? Would I remember my skills? What if someone crashes will I know what to do? If everything is okay will I know what to do? A year of nursing school was coming to an end and suddenly I felt like I had forgotten everything. I arrived at the hospital much earlier then I needed to and tried to relax. I took a deep breath and headed in to the ICU trying to pretend that I wasn’t terrified. I obviously…

Clinical has progressively gotten better for me because I gained more hands-on experience. My greatest challenges so far have been thinking appropriately in a quick amount of time and taking initiative. All of my challenges will make me stronger and more confident in the Long-Term Care setting. More so, I’ve had accomplishments in the setting which have made me more sure of myself. I believe it’s important to learn from the challenges, rather than thinking of it as a doom. Nurses are going to be facing difficult situations, but we have to learn to think appropriately and do the best we can.…

Once I was done I helped to take some resident to their room and locate a room for a resident who is trying to find her room. The new skill that I experienced this week was head injury assessment. It was definitely very useful to participate in that skill plus gaining more experience. One of the weaknesses I finds that I do not use a good body mechanic when caring for a resident which is extremely bad because you need to always have a good body mechanic to protect your back or any part of the body. Therefore, my plan for next week uses a good body mechanic when I do any kind of care. My shift also included help with feeding lunch meal and after that taking some resident back to their room for nap depending on their will. I also made some beds near the end of my shift as well as answering care bells and checking on residents from time to time. Some resident wanted to attend the music activities so I took some of the them to attend and enjoy music.I am looking forward to next week to learn an d experience new skills and well as gain my experience on my current…

Rotation Day 8 today I’m in processing it was a slower day than yesterday so the processer had me help with a few things. They also asked me if I could go with a phlebotomist to drop some blood to the Operation room. When we got there I noticed read lines on the floor meaning we could not pass until we had full gowns. We waited to see if there was someone we could flag since there was no one in the front office. A person came in and we told her we had a delivery she took it and we left. When we got back they had a request to draw a patient in the SSI department I didn’t know where this was so the phlebotomist asked if I wanted to go check it out so I went. I didn’t know it we had to drive since it was in another location. We went in and the patient was in the recovery room she was alert at the moment and recognized the phlebotomist right away.…

They had already attended 74 patients before I arrived in the afternoon to begin my job shadow. Since the workers have a lot of experience, it was easy for them to go through the list of patients quickly. It also surprised me that they had a portable x-ray, and it was the first time I got to see one. The x-ray was used for patients who cannot stand or leave their rooms due to problems they have. Being able to observe an x-ray being done in a patient's room was very interesting.…
