* A 68-year-old male presents to the office complaining of pronounced weakness on the right side of his body and slurred speech for the past 24 hours. Based on the examination, the physician orders an MRI to investigate a possible transient ischemic attack.
The diagnosis category I would choose in this case would be Symptoms, Signs, and Ill-Defined Conditions—Codes 780–799. I would choose this category because the patient came in complaining of various symptoms so the physician ordered an MRI to investigate for a possible transient ischemic attack which is not a diagnosis. * A 44-year-old male presents to the office complaining of intermittent chest pain. The physician orders an EKG to rule out a possible cardiac event.
The diagnosis category I would choose in this case would be Diseases of the Circulatory System—Codes 390–459. The patient is complaining of chest pains and feels the patient might be having a cardiac event you would use these codes because they deal with the circulatory system. * A 72-year-old diabetic female exhibits a non-healing wound on her left foot.
The diagnosis category I would choose in this case would be Diseases of the Skin and Subcutaneous Tissue—Codes 680–709. The reasoning for this is that the patient’s problem is a wound that is not healing on her foot. The fact that the patient suffers from diabetes is not a diagnosis of why the wound is not

The term first-listed diagnosis/condition is used in the outpatient setting in lieu of principal diagnosis, once again because of the timing. Often, diagnoses are not established at the time of the initial encounter in the outpatient setting and it may take two or more visits prior to a confirmed diagnosis. The documentation to support the reason for the visit should describe the patient's condition, using terminology that includes either specific diagnoses and/or symptoms, problems, or reasons for the encounter. In the instance where a discrepancy is discovered, determining the first-listed diagnosis per the coding conventions of ICD-9-CM, as well as the general and disease-specific guidelines within ICD-9-CM, will have precedence over the outpatient guidelines.…

The patient is a 49-year-old right-handed woman who on Wednesday, last week, had 2 episodes of altered functioning within a 5- to 10-minute period. She states that she was walking down a flight of stairs, and she knew she was at the bottom, but she took another step as if she had 1 more step to distend. She said she felt that she knew that there was not another step, but she took it anyway and felt somewhat confused as to why she had done it. Within the next 5 to 10 minutes, she was shopping at a grocery store, looking at a jar of pickles, wondering why she was looking at that. She turned to the left, and she felt as if her whole left side was shaking, her vision was slightly blurred. The episode lasted only a few seconds. She became very anxious and nervous and again she felt somewhat confused. She took her blood sugar and it was 55. She has had lower blood sugars without reaction, and her typical hypoglycemic reaction is diaphoresis and weakness. She had no accompanying or following headache. She has not had any subsequent symptoms. She had no associated symptoms. She did not lose consciousness, or have any weakness or paresthesias.…
