The Universal Troubleshooting Process
Regardless of how complex your particular computer or peripheral device might be, a dependable troubleshooting procedure can be broken down into four basic steps (Fig. 4-1): define your symptoms, identify and isolate the potential source (or location) of your problem, replace the suspected sub-assembly, and re-test the unit thoroughly to be sure that you have solved the problem. If you have not solved the problem, start again from Step #1. This is a “universal” procedure that you can apply to any sort of troubleshooting—not just for personal computer equipment.

Cognitive skill errors may lead to the ignorance of patient 's needs and rights. Considering patient and family members as part of the treatment plan is necessary because they know how they are doing better than anyone else. For example, when Helen expresses her concern of Lewis’ pain, nurse told Helen that the pain is gas and Lewis needs to move around although Lewis is having slight fever, cold to touch, and pain is five on scale of five (Helen Haskell, 2009). This premature closure has led to fail to treat Lewis properly. Another example is when Helen calls nurse many times to complain about Lewis’ pain, she tries to convince Helen that Lewis is not walking enough to relief the gas (Helen Haskell, 2009).Availability bias and responsiveness bias may also affect nurse 's belief on the cause of pain and lead to wrong diagnosis. Nurse may have seen many patients who have gas pain prior to Lewis and they use their past experience of gas pain to diagnose Lewis’ problem without considering other possibilities. The nurse also seeks data relevant with gas pain while she ignores the data relate to internal bleeding. The other failure of patient-center care in Lewis story is no one notices the deadly side effect of Ketorolac although it is stated clearly about the…

    1) FC&C made a number of mistakes at the management level that contributed to the failure of this system. First, it sent an intern, by himself, on his first project. It is unreasonable to think that Blake, after only two weeks of orientation, had the skill set necessary to get the job done by himself. They should have sent in some kind of support team for Blake to fall back on. Second, they originally only slated one week for this assignment. It make sense that FC&C would want to provide the Royal Hotel with a timetable for completion, but they did so before the project even started and set expectations unreasonably high. It would have been a better plan to tell Royal Hotel that they would have a better timeline available one the analysis phase was completed. Third, FC&C pulled their intern before the project was completed and replaced him with a different intern. It seems that if this firm wanted any kind of continuity it may have been better suited leaving Blake in to close out the job.…

A sophisticated continuous quality improvement process should involve the clinical employees as well as the senior medical staff. “Leading an organization refers to an individual’s ability to galvanize resources and motivate employees to work collectively to further organizational goals, which goes beyond simply controlling day-to-day operations.” (O'Connor, 2009) Continuous quality improvement cannot function properly without effective leadership and managers who have a dual role in managing staff as well being a part of the leadership of the organization as a whole. By definition, continuous quality improvement involves employee empowerment and interdisciplinary teams. By only utilizing the senior nurse manager in monthly CQI meetings, clinical staff, which are a very integral part of the overall…
