Dugway, UT 84022


1. PURPOSE: To establish policies and procedures for patients to obtain prescription eyewear.

2. SCOPE: This SOP is applicable to all personnel assigned or attached to the Optometry section and to all eligible patients seeking optical services through the Optometry section.

d. Optical Laboratory Bulletins

4. PROCEDURES: Provision of prescription eyewear is an integral part of the optometric services provided by this facility. Individual patients will obtain their eyewear based upon the following factors.

a. Categories:

(1) Active duty military will have their spectacle prescription filled in standard military issue frames and lenses appropriate to their MOS and duties. One pair of “civilian-style” frames may be issued annually as part of the Army’s Frame of Choice Program.

(2) Retired military personnel may have their spectacle prescription filled in standard issue frames and lenses. (Unfortunately, at this time, the Army does not provide FOC for retired personnel)

(3) Other DOD sponsored individuals who require military issue eyewear will have eligibility verified by the Patient Administration Division prior to submission of DD Form 771, Eyewear Prescription to the servicing optical fabrication laboratory for filling at government expense. All spectacle orders originating from the Optometry clinic will be returned to the clinic before dispensing to maintain accountability.

(4) Dependents of active duty military, retired, and deceased military, and other eligible personnel may buy their eye wear through a military exchange concessionaire or a civilian optical store at their own expense.

b. Repair and dispensing of eyewear will only be attempted on government issue or ordered materials. This clinic will not be responsible for adjustments made to civilian frames (breakage is possible and the clinic will not reimburse).

c. DD Form 771 will be completed IAW AR 40-63. For patients who live off post the form will include their mailing address, so the eyewear can be mailed to the patient from this clinic. A phone number is required for all patients and those that live on Dugway Proving Ground will be called and notified that their glasses have been received and can be picked up at the eye clinic.

d. Patients may have their eyewear prescription verified by a technician during clinic hours. Prescriptions that have been incorrectly filled will be brought to the attention of the prescribing optometrist.

e. Each patient will receive an eyeglass prescription after a full eye exam (medical condition allowing) to obtain eyewear through any optical shop.

f. Patients with a complaint of visual discomfort secondary to new eyewear will first have the prescription verified. If the eyewear is of the correct prescription and if the eyewear still causes problems after the patient has tried to wear the new prescription for several days, then an appointment will be made for the patient to be seen again by the prescribing optometrist. Such appointments will be expedited after conferring with the prescribing optometrist. If it is determined that a change in the prescription is needed then the following guidelines will be followed:

(1) Government issued eyewear new eyewear will be ordered in the appropriate prescription.
