1. Company/Project Description a. Specific name of the company/project b. Location of company/project c. Company/project classification d. Company/project owner e. Name of main contractor if project f. Start of construction if project g. Estimated number of workers
2. General Company Safety Policy
a. Company Safety and Health Policy written on company letterhead must be signed by the highest ranking company representative who has the over-all control. This Shall serve as general guiding principle in the implementation of safety and health on work site. b. Safety Policy must comply with the DOLE minimum safety and health requirements.
This include but not limited to the ff: . Registration(rule1020) . Report Safety Committee Org. under (rule 1040) . Notification of accidents and occupational illnesses under (rule 1050) . Annual work accident/illnesses exposure data report(rule 1050) . Approved application or mech/elec equipment(rule 1070 and 1160) . Annual Medical Report
3. Composition of Safety and Health Committee
a. Submit the proposed structure and membership or the safety and health committee, it must comply with the mim. requirement of rule 1040 of OSHS b.Include the proposed frequency of meetings of the safety committee
4. Site Safety and Health Personnel a. First aider must be certified by the NRC b. Safety officers must satisfy the requirements of BWC-DOLE c. All doctors and nurses must complete the BOSH course or equivalent
5. Duties and Responsibilities of the Safety Officer a. As stated in the OSHS b. Specify on how the Safety Officer will effective in his/her job
6. Worksite Safety and Health Promotion and Continuing Information Dissemination a. Specifics on how employees shall be briefed on safety and health policies b. Specifics on what kind trainings and seminars will be given to employees c.
