Two days after the vessel sailed out from port, the boson who is the deck hand in charge opened the forward store and started the ventilation fan. Immediately an explosion[Fig-5] occurred and the boson was killed. Also, it ruptured the forward store badly and deformed the forward shape. [36]
What Went Wrong (Critical Factors):
The de-humidifier consists of several valves which was connected to cargo line. In the normal procedure the vessel must remove the spool connection from the cargo line and the de-humidifier. In the above case, after investigation it was noted that de-humidifier spool connection was not removed and while loading cargo then the cargo gasses was passing through the spool connection the de-humidifier. As a result the forward store was rich of flammable gasses except ignition. The forward ventilation fan provided the ignition source. As soon as fan started then the explosion happened. [36]
The boson also, ignored the strong smell of cargo as he found the smell as soon as he opened the door of forward store. He was ignorant to inform senior staff instead of running the fan

The modern ship already designed for that. All chemical tankers have separated cargo tanks which is completely sealed. If the chemical not expose to weather then there will be no toxicity. The work force must maintain leakage free operation so that no vapor or liquid releasing outside. However if need to open some system as a part of operation then extreme caution must be taken with proper PPA and handled by the experienced person. Most of the toxic chemical vapor are heavier than air which can be trapped at various work place. So proper gas monitoring equipment must be use while working in the vicinity. Gas monitoring equipment will give automatic alarm to warn the workforce if toxic vapor is outside the allowable limit.
