The safety of the surgery is critical issue to discuss because the learning from the mistakes can improve the patient safety. The surgery is an essential part of health care process, which prevents life loss in many critical cases. Although the surgery has saved life, it also accompanied with considerable risk of death (Weiser et al., 2008).
It is noticeable that a teamwork and effective communication increase the positive outcome; moreover, a skilled professional team reduces the rate of adverse events. In proper communication between health care providers can cause the adverse result. Many studies have shown an improvement by the member of care team regarding collaboration, teamwork and communication but there studies not objectively assessed

Using Structure, Process and Outcome (SPO) framework to identify how to improvement service process selected
Donabedian's framework or model based on : structure, process, and outcome. (Donabedian, The Quality of Care How Can It Be Assessed?, 1988) (Donabedian, The Definition of Quality and Approaches to Its Assessment, 1980). Structures like the organizational structure, staff and machine. Second factor is Process, which describes to how structure is placed into practice or the technique of care delivered, such as medical care, surgical process. And the third factor is Outcome, which is a result of process like results of therapy, mortality rate and length of stay. SPO model might be used to explain and assess single quality systems or to compare different quality systems. (Kunkel, Rosenqvist, & Westerling, 2007).
We developed SPO framework the area of services, which need improvement in Mildred’s case. Below is the figure 1.1, which shown structure process and outcome in Mildred’s case from preoperative stage to postoperative stage and what actually happened in Mildred’s case video and what is not showed in video, which require improvement. By comparing the result between two columns, it is easy to describe the areas where the quality of health care services can be
