Review exactly how both the individual service users featured in your case studies, and the professionals who deliver their care, benefit from a holistic approach to planning. You need to make the links between planning and the delivery of support clear. [M1]
Consider at least three reasons for taking a multi-agency approach to planning support, analysing each of your reasons and justifying your explanations.
