Unit 7 P4 Health And Social Care Essay
This assignment will be looking at how the practitioners and agencies involved in the care pathway work with one another to provide the care needed for diabetes and dementia.
A Multi-disciplinary team is ‘’a group of health care and social care professionals who provide different services for patients in a co-ordinated way. Members of the team may vary and will depend on the patient's needs and the condition or disease being treated.’’ This means that these health care professionals work together as part of a team in order to provide care for their patients. The MDT support care by working together and helping the individual to improve their condition. They recommend agencies to individuals who might be suffering from physiological disorders. For example if someone is suffering from CHD the MDT might refer them to the British heart foundation so that they can help them with their disorder.

Coronary heart disease is the UK’s biggest killer and the British Heart Foundation is doing its best to change that. They are also funding thousands of research projects around the UK that are fighting heart disease. (British Heart Foundation, 2014) The British Heart Foundation helps millions of people every year with up to date information about heart disease. This means that individuals are better informed about the disease; for example if an individual who was just diagnosed with CHD was referred by the MDT the British Heart Foundation they could educate them about the disorder, this will help them minimise the effects of the disorder and keep them controlled.
