Type 2 Diabetes Case Study

The implementation of Patient-Centered Medical Home (PCMH)model in all qualified community health center across the country with the relationship of type 2 diabetes. Which means this model will be use as source of providing primary care to all patients. Some of the questions that need to considered are:

(2) will the model improve health outcome; and
