The triage nurse was the bridge between the triage room and the emergency department with exclusion of ambulance emergencies or emergency drop offs. During the visit to the triage waiting room, the nurse determines the severity of the patient's emergency depending on many things, but primarily on the ESI method. The ESI is use a lot by triage waiting rooms to determine the patient acuity and the need to go into the emergency room and treat what ever the problem is. The ESI is numeric scale from one to five, one being the must urgent and five being the least active intensity. These numbers can change if the client for some reason becomes worse if originality their were a five but lost conscious, becoming a one in seconds. In another point and according to the article, Comparison of a 5-Level Triage Classification System with a 4-Level Triage Classification System as it Relates to Acuity Assignment and Predictability of Patient Outcomes, explains that using the five level triage classification was recommended the American College of Emergency Physicians and the Emergency Nurses Association as the standardize triage to help more than a level four triage classification for accurate patient outcomes. People that place themselves in an anxiety mode have limited reasoning skills when they come in into the triage waiting room and want attention now without knowing that their care does not compare with other patients who actually need that intervention. Fortunately, the use of the ESI and clinical judgment can hep distinguish between a patient who exaggerates and a patient who requires emergency intervention. Because the ESI is really useful in many places, especially in the ER and triage, nurses use this method to help them guided throughout the patient's worries and determine which resources can help the patient fell better. As long as a nurse knows how to differentiate between a severe urgency and a least urgent situation, then that nurse is doing a great job to help the most wanted and leave those people who need help but not as much as those who needed now for
