Strategies for Ongoing Performances

Trial-and-error or jumping to solutions without sufficient study

 Extensive study of a problem which can lead to "analysis paralysis“
 Best practices by adopting someone else's success
 Top-down - leaders decide what changes are made

Identify the scope
• Service
• Customer/Patient Needs

Identify functions of
the scope
• What is the purpose?
• What do our
customers/patients
expect it to do?

Potential risks
• Identify potential ways it
could reoccur.
• Rewrite function to show
failures.

DECISION MAKING
PROCESS

Controls
• Tests
• Procedures
• Mechanisms

Root Causes

Consequences

• Determine all possible
causes
• Determine occurrance
rating

• Potential effects of the
failure

Calculate the Risk

Implement Decision

• Rank potential failures
• Identify actions to lower
risks

• Implement and evaluate
• Make necessary changes
